1、Certificate for reporting loss from the local police station(including the basic personal information, full name on the passport and passport number)

2、Accommodation registration paper for foreigners(including the basic personal information, full name on the passport and passport number)
3、Copy of the first page of passport and visa(the photos of them need to be printed out)
